
PATIENT REGISTRATION FOR PATIENT PORTAL
All On^Point patients, current and new patients, are required to register as a NEW PATIENT in On^Point's new electronic health record (called "CERBO") by clicking on this link to set up their patient portal and provide contact information and medical history:
ALL PATIENTS: Registration is a 2 step process:

STEP 1: Register and set up your patient portal. IMPORTANT: There has been some confusion for patients after they click on the link above or below to register and create their patient portal. What you will see first are fields asking for your username and password. You do not have these yet. Ignore those fields and instead look towards the bottom of the page where it says: “New Patient Registration” and click on that phrase to start the process.

STEP 2: Sign in to your patient portal and complete the appropriate forms on the portal:
For New Patients: Once you've registered, log into your portal and click on the "Forms and Questionnaires" Tab. Complete the NEW PATIENT forms. THEN... click on the tabs for Your Medications, Your Supplements, and Your Allergies and provide that information.

For Current Patients: Once you've registered, log into your portal and click on the "Forms and Questionnaires" Tab. Complete the Current Patient Forms. THEN...click on the tabs for Your Medications, Your Supplements, and Your Allergies and provide that information.
